Cholecystitis

INTRODUCTION

Cholecystitis means Inflammation of the Gall Bladder. When it's acute, it's painful and in gallstones, we should always not wait "till the patient's strength is exhausted. Acute and Chronic is either calculus or less commonly calculus. Since the clinical picture of those two entities differs, they're discussed separately.

  1. Acute Cholecystitis
  2. Chronic or old Cholecystitis


ACUTE CHOLECYSTITIS

Acute Cholecystitis is caused by a gallstone, which obstructs the gallbladder's outlet. Its spontaneous dislodgement leads to so-called biliary colic, while persisting impaction of the stone produces gallbladder distention and inflammation. Later, gut (intestine) bacteria invade the inflamed organ and infection supervenes.


CHRONIC CHOLECYSTITIS

It is the condition where the patient carries for while, even years. Sometimes feels pain at the inner angle of the right scapula(back cut of beef) related to severe acidity. It's another causation of Gallbladder cancer.


SIGN AND SYMPTOMS
  1. In acute cholecystitis, there's severe unbearable pain at the right inner side of the 9th ribs, called Murphy's sign.
  2. Nausea (tendency to vomiting)
  3. Acidity
  4. Constipation
  5. Rarely bleeding from the anus
  6. Indigestion, especially fatty foods
  7. Less thirsty
  8. Sometimes poly chromatic stool means stool with different color
  9. Blotted abdomen, means full of gas, especially upper abdomen
  10. Jaundice
  11. When the gallbladder fills with stone, then it's called CHOLELITHIASIS, and it needs treatment as yearly as possible.


ACALCULOUS CHOLECYSTITIS

This is a manifestation of disturbed microcirculation in critically ill patients. Although of multifactorial causation (e.g. prolonged fasting, administration of total parental nutrition, etc.)
the common pathogenic pathway is perhaps gallbladder ischemia, mucosal injury, and secondary bacterial invasion. Acalculous cholecystitis may be a life-threatening condition developing during a significant illness, e.g., following an operation or after severe injury. Stones may occasionally be present within the acutely inflamed gallbladders in these circumstances, but are probably etiologically irrelevant.


INVESTIGATION
  1. Ultrasonography of the whole abdomen
  2. Oral Cholecystogram
  3. Liver function test
  4. Scan of abdomen


HOMEOPATHIC MEDICINES

CINCHONA OFF 6 may be excellent medicine for gallstone colic/pain

CHOLESTERINUM 30 is another good medicine for it, and it's an influence to dissolve the cholesterol stone

BELLADONNA 30, when the patient feels much pain. Provides it frequently.

CHELLEDONIUM M 6 very needful medicine for any sort of gallstone.
