Definition

This is a condition of sudden onset in which the pre-existing tight prepuce has been retracted above the glans but can not be reduced back over it. It causes partial strangulation of the glans penis.

About

The glans become edematous, congested, and may rarely become gangrenous.

Unrelieved retraction will impede venous blood flow and lymphatic obstruction of the distal part of the penis to cause edema and enlargement of the glans penis. This will also create a color of edematous tense all-around marking reduction of prepuce more difficult.

Common in young men between 15-35 years of age. Past history of phimosis or tight prepuce.

Treatment

In early case manipulative reduction.

The para-phimosis is reduced by pulling the foreskin forward by the index and middle finger and the glans is pushed backward by thumbs. Local warm compress, multiple punctures of the para-phimosis, injection of hyaluronidase may help in reducing the edema and help reduction.

If these measures fail, a dorsal slit divides the constriction band after which reduction is possible.

If necessary, circumcision may be performed in the same sitting.

For dogs, the same measure has to be taken.
